Triage Nurse - RN (Full-Time)

Ohio's Hospice is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Full-Time Triage Nurse
. This role is ideal for an RN who enjoys providing clinical support, coordinating patient care, and helping patients and families receive timely, compassionate service throughout their hospice journey.

  • Full-Time
  • Hours: 6:00 AM - 6:30 PM
  • Weekend Rotation: Every other weekend
About the Role

The Triage Nurse - RN provides clinical support to hospice patients and families through our Call Center. In this role, you will assess incoming calls, prioritize patient needs, coordinate appropriate clinical services, and communicate with members of the interdisciplinary team to ensure continuity of care.

Your clinical judgment, assessment skills, and compassionate communication will help ensure patients receive timely care focused on comfort, dignity, and quality of life.

Key Responsibilities
  • Receive and triage patient and family calls based on clinical urgency and individual needs.
  • Provide professional, compassionate customer service and clinical guidance.
  • Assess patient needs and coordinate appropriate services according to the established plan of care.
  • Communicate changes in patient condition or needs to the appropriate clinical staff.
  • Coordinate changes in level of care with schedulers and other departments as needed.
  • Document assessments, interventions, observations, and communications accurately and promptly in the electronic medical record.
  • Maintain accurate call logs and ensure pertinent information is communicated effectively to the patient's interdisciplinary team.
  • Support continuity and coordination of care to promote comfort, dignity, and a peaceful end-of-life experience.
Required
  • Current Registered Nurse (RN) license in the State of Ohio.
  • Current CPR certification.
  • Valid driver's license with a safe driving record.
  • State-required minimum auto insurance coverage.
  • Strong documentation and computer skills.
  • Excellent assessment and clinical judgment skills.
  • Experience with pain and symptom management.
  • IV and vascular access device skills.
  • Ability to perform basic mathematical calculations, including whole numbers, fractions, decimals, ratios, rates, and percentages.
Certification
  • CHPN certification required or must be obtained within two years of employment.
