Ambulatory Nurse, Nurse Navigator – Gastroenterology

• Perform treatments and administer medications
• Assist with procedures performed without anesthesia using aseptic technique
• Assist with employee education and orientation
• Provide patient, family, staff, and community education
• Involve families/significant others in health-care planning and decisions
• Delegate and evaluate care provided by assistive personnel
• Collaborate with medical, nursing, social services, and other staff
• Ensure comprehensive care plans are implemented for pediatric and applicable adult patients
• Support the GI Division in delivering high-quality care to patients with chronic disease
• Coordinate individualized plans of care with the IBD Center Medical Director, families, and health-care team
• Perform care coordination, case management, and transitions of care
• Communicate care plans to appropriate stakeholders
• Ensure care is delivered in the appropriate setting efficiently and cost-effectively

• Bachelor of Science in Nursing (BSN)
• Registered Nurse (RN) license in the state of Ohio
• 1–2 years of nursing experience
• CPR certification
• Proficient computer skills
• Strong customer service and outreach skills
• Proficient in Microsoft Office Suite, including Word, Excel, and PowerPoint (preferred)
• Experience operating and troubleshooting audiovisual (AV) equipment (preferred)
• Strong understanding of growth and development from infancy through adulthood
• Ability to travel to other locations and attend conferences as needed

Demonstrates expertise in patient care coordination, case management, and the implementation of comprehensive care plans for pediatric and adult patients. Proficient in collaborating with multidisciplinary teams and providing education to patients, families, and staff.
