Role Overview We are seeking a highly skilled Epic Healthy Planet Analyst to drive population health initiatives and optimize value-based care workflows. This role focuses on translating clinical and social data into actionable insights and operational workflows that improve patient outcomes and organizational performance.

The ideal candidate will have deep technical expertise in Healthy Planet build, with a specific focus on Social Determinants of Health (SDOH) workflows, AI-enabled Risk Adjustment, and the operationalization of care gap closures.

Key Responsibilities 1. SDOH Workflow Optimization

  • Design and implement end-to-end Social Determinants of Health (SDOH) workflows, moving beyond simple screening to active follow-up and closed-loop referrals.
  • Configure clinical tools (navigators, flow sheets, and Best Practice Advisories) to trigger specific intervention workflows based on identified social risks (e.g., food insecurity, transportation barriers).
  • Partner with case management and community resource teams to ensure SDOH data is actionable within the clinical workspace.
2. Risk Adjustment & AI Integration
  • Lead the implementation and optimization of Risk Adjustment workflows to ensure accurate Hierarchical Condition Category (HCC) coding and recapture.
  • Specialized Focus: Implement and tune Epic's new Pre-Visit AI for Risk Adjustment, ensuring AI-generated suggestions are presented seamlessly to clinicians within their native workflows.
  • Audit and refine "suspecting" logic to improve the accuracy of patient risk scores without increasing clinician burnout.
3. Operationalizing Care Gap Closure
  • Translate population health metrics into frontline operational tools, including the configuration of Reporting Workbench reports, Radar dashboards, and Healthy Planet Work queues (WQs).
  • Build and maintain "Care Gaps" within the Healthy Planet module, ensuring logic is aligned with HEDIS, MIPS, or specific payer contract requirements.
  • Design outreach workflows (Bulk MyChart messages, letters, and call lists) to engage patients with outstanding care gaps.
Technical Requirements
  • Certification: Active Epic Healthy Planet Certification is required.
  • Experience: Minimum of 3-5 years of Epic build experience, with at least 2 years dedicated to Healthy Planet.
  • AI/Analytics: Familiarity with Epic's Cognitive Computing platform and the integration of AI models into clinical workflows.
  • Reporting: Proficiency in Reporting Workbench, Radar Dashboard configuration, and a strong understanding of how Registry data flows into operational tools.
  • Regulatory Knowledge: Working knowledge of Value-Based Care (VBC) models, HCC risk adjustment, and SDOH reporting standards.

Reporting:
Proficiency in Reporting Workbench, Radar Dashboard configuration, and a strong understanding of how Registry data flows into operational tools.

Regulatory Knowledge:
Working knowledge of Value-Based Care (VBC) models, HCC risk adjustment, and SDOH reporting standards.
