Job OverviewWe are seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide in‑home nursing care to pediatric patients in the Ponte Vedra, FL area. Responsibilities include delivering comprehensive nursing services, maintaining accurate patient records, collaborating with physicians, and ensuring compliance with Family First policies and state regulations.
Key Responsibilities- Provide care based on licensure/certification within scope of practice.
- Maintain updated clinical records and meet documentation deadlines.
- Evaluate the effectiveness of nursing services on an ongoing basis.
- Consult with physicians regarding changes in patient care plans and report all changes to the nurse supervisor.
- Mentor new employees during patient‑specific orientations.
- Educate patients and caregivers on care needs.
- Participate in the Quality Assurance, Performance Improvement program as requested.
- Complete all mandatory training and in‑services.
- Submit required personnel file documentation promptly.
- Adhere to compliance, ethics, confidentiality, and professional communication standards.
- Perform other duties as assigned.
- LPN/LVN Nursing Degree and Unencumbered LPN/LVN License (or Multi‑State license through ENLC).
- At least 1 year of demonstrated nursing experience.
- Valid driver license with proof of insurance (required in some states).
- Clear background screening.
- Working knowledge of Nurse Practice Acts.
- Valid and current CPR card/certificate and in‑person instructor‑led skills check.
- Clear Hep B and TB screening as required by state.
- Experience with pediatric patients.
- Experience with post‑operative or long‑term care of children with complex medical needs.

Work is performed in patients’ homes. Must be able to lift up to 35 pounds without an assisted lifting device. Frequent use of office machines and telephone is required.
