• Provide care within the scope of licensure/certification.
• Maintain updated clinical records and meet documentation deadlines.
• Evaluate the effectiveness of nursing services for patients and families.
• Consult with physicians regarding changes to patient care plans and report all changes to the nurse supervisor.
• Mentor and coach new employees during patient‑specific orientations.
• Educate patients and caregivers on care‑related topics.
• Participate in the Quality Assurance, Performance Improvement program (QAPI) as requested.
• Continuously improve nursing skills.
• Attend mandatory training and in‑services.
• Submit required personnel file documentation promptly.
• Adhere to compliance and ethics expectations.
• Maintain patient and family confidentiality.
• Demonstrate professional, supportive, and responsive interpersonal communication.
• Perform additional duties as assigned.

Required

• LPN/LVN nursing degree.
• Unencumbered LPN/LVN license in the state of employment or a multi‑state license via the Enhanced Nurse Licensure Compact where recognized.
• At least one year of demonstrated experience.
• Valid driver’s license with proof of insurance in applicable states.
• Clear background screening.
• Working knowledge of state Nurse Practice Acts.
• Valid CPR card and in‑person instructor‑led hands‑on skills check.
• Clear Hepatitis B screening in applicable states.
• Clear tuberculosis screening in applicable states.

Preferred

• State‑specific qualifications, e.g., for Florida and Illinois: no driving prerequisites;
  Hepatitis B screening completed during onboarding.
• For Pennsylvania: additional background screening for child abuse and FBI check; TB screening with two‑step PPD or proof of two‑step administration within the last year.

• Proficiency in reading and communicating in English, both verbally and in writing.

• Work performed in patient homes; not substantially exposed to adverse environmental conditions.
• Training may occur in an office setting.
• Must lift up to 35 pounds without an assisted lifting device.
• Frequent use of telephone, computer, and printer.
