RN Nurse, Private Duty Nurse
Job Overview
Administer skilled nursing care for clients of all ages in their residence. Assess home health and hospice patients to identify physical, psychosocial, and environmental needs. Implement, develop, document, and evaluate the Plan of Care (POC) for quality and continuity of care. Provide education to patients and families regarding care, medications, and self‑care practices. Supervise and provide clinical direction to Home Health Aides and nursing staff.
Perform on‑call responsibilities and communicate effectively with patients, families, and team members. Monitor compliance with third‑party payer requirements and facilitate referrals to appropriate agencies.
Shift: Monday to Friday until shift is complete; on‑call required if requested. This position requires working in home settings; local travel is necessary.
Qualifications- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- Current state license as a Registered Nurse.
- CPR certification.
- CHPN certification (preferred).
- Benefits are available to full‑time employees after 90 days of employment.
