You will participate in multidisciplinary rounds, educate patients and families, assess readmission reasons, and develop discharge plans across inpatient, observation and ED settings.
Required:
Associates of Nursing; 2+ medical/hospital nursing experience;
Bachelors in Nursing preferred. CCM/ACM preferred.
