Responsibilities:

• Provide direct patient care to a diverse population of residents in a long-term care setting
• Conduct initial and ongoing assessments to identify resident needs and develop personalized care plans
• Administer medications, treatments, and therapies as prescribed by physicians and ordered by physicians' assistants
• Monitor and report on resident vital signs, conditions, and behaviors
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Communicate effectively with residents, families, and healthcare professionals to ensure seamless care transitions and coordination
