PRN Family Nurse Practitioner

Job Description

CHFNP is seeking a licensed Family Nurse Practitioner to join our team on a per diem basis, providing primary and preventive care to patients in Palm Coast and Daytona, Florida. This role supports CHFNP's mission of delivering accessible, high-quality care to underserved, homebound, and elderly patient populations across home, and assisted living facility (ALF). Assignments and shifts are scheduled flexibly based on patient volume and coverage needs in each service area.

Roles & Responsibilities

Clinical Care

• Conduct comprehensive history and physical examinations for patients in the home, ALF, and/or office

setting as assigned.

• Evaluate, diagnose, and manage acute and chronic conditions within FNP scope of practice.

• Develop and implement individualized treatment and care plans, including medication management.

• Order, perform, and interpret diagnostic tests (labs, EKGs, Holter monitoring, point-of-care testing) as clinically indicated.

• Coordinate care with collaborating specialists (cardiology, urology, and other referral partners) as

needed.

• Charts must be completed within 24-48 hours.
• Deliver care in compassionate care with empathy, kindness and respect.
• Background check is required as part of hiring process.
