What is Take Your Therapist Home?In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is a one-of-a-kind program where you as the clinician work directly with patients in the SNF and home health, providing a continuity of care allowing you to have a greater impact on patient outcomes!
You will be primarily in the home health setting with dedicated time and caseload support for the SNF. You will have a localized coverage area in the surrounding area of the SNF. No more ever-expanding home care coverage areas. Your schedule is based on a traditional work week.

26001 Ford Road, Dearborn Heights, MI 48127. This is a hybrid role providing Home Health therapy services in conjunction with SNF based therapy services. Home Health geographic area is the vicinity around our Dearborn Heights Facility.
Home care services are paid per visit, and SNF services are paid at an hourly rate.
ScheduleFull Time Monday–Friday.
Required Qualifications- Successful completion of an accredited Occupational Therapy Assistant program.
- Licensure as required by the State of Michigan.
- Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
- Initiative and flexibility in the work environment.
- Ability to react effectively to the needs of patients and their families.
- Physical ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
