JOB POSTING Adult Transition Classroom Teacher - Moderate Cognitive Impairment Program Summary
To provide high-quality transition-focused instruction and programming for students ages 18-26 with moderate cognitive impairments whose educational needs are best met in a center-based Intermediate School District program. The teacher is responsible for planning and implementing individualized educational programming that supports post‑secondary outcomes in the areas of employment, independent living, community access, and functional skill development in accordance with each student’s Individualized Education Program (IEP).
Qualifications- Maintains attendance and evaluation records for classroom students.
- Develop and implement transition-focused instruction in:
- vocational skills
- employability
- functional academics
- daily living skills
- communication
- recreation/leisure
- and community participation
- Plans and supervises community-based instructional activities and vocational experiences in school and community settings.
- Collaborate with local employers and community agencies to create vocational and community access opportunities.
- Evaluates student’s progress based on work output and observation, and determines goals and objectives.
- Conduct transition assessments and develop measurable post‑secondary goals.
- Facilitate student participation in job training, volunteer opportunities, and independent living activities.
- Coordinate with outside agencies including:
- Michigan Rehabilitation Services (MRS)
- Community Mental Health (CMH)
- adult service providers
- transportation providers
- guardians/families
- Maintain documentation related to transition services, employability skills, and community experiences.
- Plans individualized educational programs designed to meet student’s needs and state and federal laws.
- Establishes a structured classroom to ensure the safety of each student.
- Leads and provides direct oversight of classroom paraprofessionals.
- Maintains professional, positive, and productive relationships with student’s parents, fellow teachers, and other ISD personnel.
- Refers students for observation to members of the support staff which included Speech Therapists, Physical Therapists, Occupational Therapists, School Psychologists and School Social Workers, when appropriate and necessary.
- Effectively manages students’ health, personal care, and safety, including health care plans.
- Completes Medicaid and other paperwork as appropriate.
- Ability to operate district vehicles and transport students when needed.
Ability to safely assist students with mobility, personal care, and behavioral support needs as required.

Salary RangeCompetitive pay and benefits as outlined in the C.O.O.R. Educational Association bargaining agreement.
Hours to be WorkedMinimum of 7 hours per day
EvaluationPerformance on this job will be evaluated annually as required.
