Social Work Director

Allegria Village is seeking a compassionate, experienced, and dedicated Social Work Director to join our skilled nursing team. This is an excellent opportunity for a social work professional who is passionate about advocating for residents, supporting families, and helping ensure a high quality of life for older adults.

The Social Work Director leads the social services function within our skilled nursing community and ensures residents receive comprehensive psychosocial support throughout their stay. This position works closely with residents, families, nursing staff, physicians, rehabilitation professionals, and other members of the interdisciplinary team.

• Direct and oversee social work services for skilled nursing residents.

• Complete psychosocial assessments and develop individualized plans of care.

• Provide counseling, emotional support, crisis intervention, and advocacy for residents and families.

• Participate in care plan meetings and interdisciplinary team conferences.

• Assist residents and families with adjustment to the skilled nursing environment and changes in health or functional status.

• Coordinate discharge planning and collaborate with residents, families, healthcare providers, and community resources.

• Assist with transitions between levels of care and connect residents with appropriate services and resources.

• Address resident concerns and grievances in a timely and professional manner.

• Maintain accurate and timely documentation in accordance with regulatory and organizational requirements.

• Support resident rights, dignity, choice, and quality of life.

• Collaborate with nursing, rehabilitation, activities, admissions, and other departments to promote person-centered care.

• Provide leadership, guidance, and support to social services staff.

• Participate in quality improvement initiatives and regulatory surveys as appropriate.

• Maintain knowledge of applicable federal and state regulations governing social services in skilled nursing.

• Bachelor's degree in Social Work required; MSW preferred.

• Current Michigan social work licensure or appropriate credential required.

• Experience working with older adults, long-term care, skilled nursing, rehabilitation, or healthcare populations strongly preferred.

• Previous supervisory or leadership experience preferred.

• Strong knowledge of psychosocial assessment, care planning, discharge planning, and community resources.

• Excellent communication, organization, documentation, and interpersonal skills.

• Demonstrated ability to advocate effectively for residents while collaborating with an interdisciplinary team.
