Overview
The Graduate Nurse (GN) is a member of the patient care delivery team. Under the supervision of a registered nurse, the GN engages in patient care activities, including patient/family education, for a group of patients and their families through communication and collaboration with all team members, ancillary services and support services.
It is recognized that specific patient care tasks may vary across units or service lines based on patient population. Individual units may appropriately add to or delete from the duties defined in the Graduate Nurse job description when approved by nursing administration.
Responsibilities- Collaborates and cooperates with care team members to ensure quality of care and promote productive employee relations. Demonstrates commitment to continuous improvement, teamwork, and the mission/values of the DMC, adhering to Customer Service Standards.
- Under the supervision of a registered nurse, coordinates patient care activities for an assigned group of patients and assumes the role of care coordinator for uncomplicated patients; utilizes clinical pathways and protocols; incorporates discharge planning into the care plan at admission; evaluates progress towards goals; modifies plans to achieve outcomes. Facilitates and manages care activities involving other departments to optimize patient outcomes and participate in cost containment.
- Under the supervision of a registered nurse, provides direct patient care as determined by patient, family and unit needs:
- Determine patient and family needs; identify goals; develop and implement plans to meet those goals; provide culturally sensitive and patient/family centered care.
- Follow through to ensure physician orders are executed according to plan.
- Document and communicate patient response to care and treatment; monitor laboratory values for therapeutic ranges; assess medication effects and side effects.
- Participate in identifying quality improvement opportunities from feedback.
- Promote patient, family, employee, visitor and unit safety.
- Identify learning needs and provide patient and family education; coordinate discharge planning and provide discharge instructions regarding treatment, follow-up care, medications, etc.
- Under the supervision of a registered nurse, collaborates with physicians and care management specialists in carrying out the nursing process by contacting and interacting with care management teams and seeking input from other caregivers in plan development, implementation and evaluation.
- The essential functions of this job include but are not limited to those described above.
Minimum Qualifications
- Graduation from an accredited school of nursing. BSN preferred.
- Must have temporary authorization to practice as a graduate nurse under the direct supervision of a registered nurse as required by law, regulation in state of practice, or policy.
- Must be licensed to practice as a registered nurse by the state of Michigan within 90 days of hire. If license is not obtained within 90 days of hire, classification may change or employment may be terminated.

- Ability to adapt practices and priorities in response to changing conditions.
- Requires physical ability and dexterity to move patients and materials, administer care, and provide emergency measures as needed.
- Basic mathematical ability for calculations.
