RoleWe are looking for a Registered Nurse Case Manager to support hospice care patients and families in Eaton County.
Responsibilities- Ensuring that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
- Assessing hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after‑hours reports, and on‑site evaluations.
- Obtaining data on physical, psychological, social, and spiritual factors that may influence patient or family health status.
- Communicating significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care.
- Counseling, instructing, and involving the patient, facility, and family in following the Interdisciplinary Plan of Care.
- Teaching hospice philosophy and pain and symptom management to staff in alternate care settings such as nursing homes, hospitals, and assisted‑living facilities.
- Providing support at the time of death and performing bereavement assessment.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
- Current State License as a Registered Nurse (RN).
- Minimum one year of experience in a clinical care setting.
- Experience in a hospice or home health environment is preferred.
- 50% travel required.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver’s license, insurance, and reliable transportation.
