Job Description: Focused on direct patient care, assessment, and collaborative team coordination in a 3 x 12 hour night‑weekend shift schedule.
Responsibilities- Conduct comprehensive patient assessments through observation, interviews, and physical examinations to gather history and vital signs, identifying physical, psychosocial, cultural, and spiritual needs.
- Develop and implement individualized care plans based on assessment data, setting specific, measurable, achievable, relevant, and time‑bound (SMART) goals in collaboration with patients, families, and healthcare teams.
- Administer medications and treatments safely, including oral, intravenous, and intramuscular forms, and perform necessary medical procedures such as wound care, catheterization, and IV therapy.
- Provide patient and family education about health conditions, treatments, and self‑care practices, delivering tailored educational materials and follow‑up plans.
- Advocate for patient health and safety by acting as a liaison between patients and the healthcare system, protecting patient rights and confidentiality, and securing necessary resources.
- Participate in performance improvement activities, engaging in unit‑based quality improvement, evidence‑based practice, and research initiatives.
- Collaborate with healthcare professionals—doctors, therapists, social workers, and other providers—to ensure coordinated and comprehensive care.
- Collect and analyze patient health data, prioritizing needs based on immediate condition and overall health status to ensure timely care.
- Document patient care activities accurately and timely in medical records, including assessments, care plans, interventions, and patient responses.
- Evaluate and adjust care plans by monitoring effectiveness and making necessary changes based on ongoing assessments and feedback.
- Perform nursing procedures such as wound care, catheterization, and specimen collection, ensuring accurate and safe implementation.
- Ensure patient safety by adhering to established policies and procedures, maintaining a safe environment, and participating in bedside shift reports.
- Perform other duties as assigned.
- Associate’s of Nursing [Required]
- Bachelor’s of Nursing [Preferred]
- 1+ prior hospital/acute care nursing experience. [Preferred]
- RN experience [Required]
- Basic Life Support - CPR Cert (BLS) [Required]
- Registered Nurse (RN) [Required]
$31.44 - $54.92
