Optimal Care is building a talent pipeline of compassionate Registered Nurses for future Hospice RN Case Manager opportunities serving Metro Detroit. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.

As a Hospice RN Case Manager you will practice the full scope of nursing, build deep, meaningful relationships, coordinate comprehensive care for your caseload, and work collaboratively with an interdisciplinary team to address the physical, emotional, and spiritual needs of patients and families.

This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes.

• Full Time:
  Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation
• Part Time:
  Three weekdays, 8 a.m. to 5 p.m., with a weekend on‑call rotation
• Per Diem:
  Flexible weekdays, Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• Minimum one (1) year experience as a nurse in a clinical care setting
• Reliable transportation, valid driver's license, and valid automobile insurance coverage

• Bachelor of Science in Nursing (BSN)
• Hospice or home health experience

• Nursing skills as defined by accepted nursing standards
• Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment
• Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
• Ability to handle multiple priorities, documentation requirements and deadlines
