Optimal Care is building a talent pipeline of compassionate Registered Nurses for future Hospice RN Case Manager opportunities serving Southwest Michigan
. Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.

About the Role

Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.

This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building report with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.

Location

  • Main Service Area Options:
    • Kalamazoo / St. Joseph / Allegan
    • Barry / Calhoun / Branch Counties
    • Allegan / Van Buren / Berrien / Cass Counties

This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

  • Full Time:
    Monday to Friday,8 a.m. to 5 p.m., with a weekend on-call rotation
  • Part Time:
    Three weekdays, 8 a.m. to 5 p.m., with a weekend on-call rotation
  • Per Diem:
    Flexible weekdays, Monday to Friday, 8 a.m. to 5 p.m., with a weekend on-call rotation

Required Qualification

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum one (1) year experience as a nurse in a clinical care setting
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN)
  • Hospice or home health experience

Essential Skills and Competencies

  • Nursing skills as defined by accepted nursing standards
  • Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment
  • Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
  • Ability to handle multiple priorities, documentation requirements and deadlines

Pay Range

$80,000 - $85,000 USD
