Job Summary:

The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes in accordance with physician orders and the patient's plan of care. The RN ensures high-quality, patient-centered care while collaborating with the interdisciplinary team to promote health, safety, and independence.

Responsibilities:
  • Provide skilled nursing care to patients in accordance with the plan of care.
  • Perform comprehensive patient assessments and ongoing evaluations.
  • Administer medications and treatments as prescribed, following safety protocols.
  • Collaborate with physicians and interdisciplinary team members to develop and update individualized care plans.
  • Monitor patient conditions and report changes promptly.
  • Perform skilled nursing procedures, including wound care, IV therapy, catheter care, and related treatments.
  • Maintain accurate, timely clinical documentation in compliance with HIPAA and agency standards.
  • Educate patients and families on disease processes, medications, and home care needs.
  • Participate in case management and care coordination to support successful outcomes.
