Case Manager RN (Remote)

Location:
Fully Remote (No onsite requirement) Michigan preferred
We’re looking for a Case Manager RN to lead care coordination for a diverse member population, delivering holistic, person-centered care across the continuum. This role serves as the primary point of contact for members, caregivers, and providers, driving outcomes through collaboration, education, and advocacy.
? What You’ll Do- Lead a multidisciplinary care team (Social Workers, Dietitians, Pharmacists, Clinical Staff, Medical Directors) to deliver integrated care
- Manage end-to-end case management processes (assessment, care planning, implementation, monitoring, evaluation)
- Develop individualized care plans based on medical, psychosocial, and cultural needs
- Coordinate services across providers, community resources, and support systems
- Identify care gaps, remove barriers, and improve health outcomes
- Educate members on health literacy, self-management, medications, and nutrition
- Support care transitions (facility ? home or other settings), ensuring continuity of care
- Arrange services such as DME, transportation, and community support resources
- Document care activities accurately and meet quality and production standards
Required Qualifications
- Active, unrestricted Michigan RN license
- Nursing Diploma or Associate’s degree (BSN preferred)
- 3+ years clinical nursing experience (acute/post‑acute/community)
- Strong critical thinking, communication, and organizational skills
- Comfortable managing multiple systems while engaging members
Preferred Qualifications
- Case management experience (managed care)
- Experience supporting members via phone and digital channels
- CCM certification (or willing to obtain within 18 months)
- CCP certification is a plus
- Care coordination & care planning
- Motivational interviewing
- Member advocacy & engagement
- Time management & multitasking
- Microsoft Office (Excel, Outlook, Teams, Word, etc.)
