Social Work Director

We are seeking an experienced and compassionate Social Work Director to support the social, emotional, and overall well-being of older adults within a senior living community.

The Social Work Director will provide leadership and oversight for social services, serve as a resource to residents and families, and collaborate closely with an interdisciplinary team to ensure residents receive appropriate support and resources.

• Provide leadership and direction for social services within the community.

• Assess residents' psychosocial needs and develop appropriate plans of support.

• Serve as an advocate and resource for residents and their families.

• Assist residents and families with transitions, changes in care needs, and adjustment to the senior living environment.

• Coordinate with healthcare providers, community agencies, and other service professionals as appropriate.

• Facilitate communication among residents, families, and members of the interdisciplinary team.

• Provide support and guidance related to emotional, social, and behavioral concerns.

• Connect residents and families with appropriate community resources and services.

• Maintain accurate and timely documentation in accordance with organizational standards.

• Provide education, support, and resources to residents, families, and staff.

• Maintain confidentiality and uphold professional and ethical standards.

• Bachelor's or Master's degree in Social Work or a related field;
  Master's degree preferred.

• Appropriate social work credentials/licensure as required.

• Previous experience as a Social Work Director in an Assisted Living, Skilled Nursing, Healthcare or post-acute care setting preferred.

• Experience supporting individuals with complex psychosocial needs and coordinating services across multiple disciplines.

• Experience working with residents, families, healthcare professionals, and community resources.

• Strong communication, organizational, assessment, and interpersonal skills.

• Ability to work independently while collaborating effectively with an interdisciplinary team.

• Compassionate, professional, and resident-centered approach to service.

The ideal candidate will bring a strong understanding of the psychosocial needs of older adults, experience navigating complex family and care situations, and the ability to coordinate effectively with multiple members of a care team. Experience in a setting serving individuals with varying levels of care needs and complex support requirements is highly valued.
