Job Description

We are seeking a compassionate and skilled Geriatric Nurse Practitioner to join our team and provide comprehensive healthcare to our geriatric patients. The ideal candidate will have a strong background in geriatric care, excellent communication skills, and a passion for improving the quality of life for elderly patients.

Responsibilities

Conducting thorough patient assessments and developing individualized care plans for geriatric patients.

Diagnosing and treating acute and chronic illnesses common in the elderly population.

Collaborating with physicians, nurses, and other healthcare professionals to ensure coordinated and effective care.

Providing patient and family education on geriatric health issues, medication management, and lifestyle modifications.

Conducting regular follow-up visits to monitor patient progress and make any necessary adjustments to treatment plans.

Participating in multidisciplinary team meetings to discuss patient cases and contribute expertise in geriatric care.

Requirements

Master's degree in Nursing with a specialization in gerontology or geriatric care.

Current Nurse Practitioner license and certification in the state of practice.

Proven experience working in geriatric healthcare, preferably in a primary care or long-term care setting.

Knowledge of geriatric syndromes, medication management, and end‑of‑life care.

Strong interpersonal skills and ability to empathize with elderly patients and their families.

Ability to work effectively in a collaborative healthcare environment.
