Southern Health Partners Inc is seeking an Experienced LPN to join our Healthcare & Medical Services team, providing quality care within correctional facilities. In this full-time night role, you'll deliver compassionate nursing care, administer medications, support chronic disease management, and respond to urgent health needs while ensuring compliance with state and facility regulations. You'll collaborate closely with an interdisciplinary team in a safe, structured environment.

Responsibilities
- Provide direct nursing care to incarcerated patients during 12-hour night shifts, including assessments, treatments, and ongoing monitoring
- Administer medications safely and accurately in accordance with provider orders, facility policies, and state regulations
- Perform wound care, dressing changes, basic respiratory treatments, and other LPN-level clinical procedures
- Monitor and manage patients with chronic conditions, reporting changes in status to the supervising RN or provider
- Triage sick call requests and respond promptly to urgent and emergent medical needs within the facility
- Assist in emergency response situations, including initial assessment, stabilization, and coordination with on-call providers or external emergency services
- Document all patient care, medications, and interventions thoroughly and accurately in the electronic medical record (EMR)
- Follow all infection control practices, medication handling protocols, and correctional healthcare regulations
- Collaborate with RNs, physicians, mental health professionals, and correctional officers to promote a safe, therapeutic environment
- Provide patient education on medications, chronic disease management, and health maintenance tailored to the correctional setting
- Adhere to facility security procedures and maintain professional boundaries at all times
- Participate in shift handoffs, ensuring clear communication of patient status, pending orders, and ongoing care needs
