Licensed Practical Nurse

Southern Health Partners Inc is seeking an Experienced LPN to join our Healthcare & Medical Services team, providing quality care within correctional facilities. Responsibilities

• Provide direct nursing care to incarcerated patients during 12-hour night shifts, including assessments, treatments, and ongoing monitoring
• Administer medications safely and accurately in accordance with provider orders, facility policies, and state regulations
• Perform wound care, dressing changes, basic respiratory treatments, and other LPN-level clinical procedures
• Monitor and manage patients with chronic conditions, reporting changes in status to the supervising RN or provider
• Triage sick call requests and respond promptly to urgent and emergent medical needs within the facility
• Assist in emergency response situations, including initial assessment, stabilization, and coordination with on-call providers or external emergency services
• Document all patient care, medications, and interventions thoroughly and accurately in the electronic medical record (EMR)
• Follow all infection control practices, medication handling protocols, and correctional healthcare regulations
• Collaborate with RNs, physicians, mental health professionals, and correctional officers to promote a safe, therapeutic environment
• Provide patient education on medications, chronic disease management, and health maintenance tailored to the correctional setting
• Adhere to facility security procedures and maintain professional boundaries at all times
• Participate in shift handoffs, ensuring clear communication of patient status, pending orders, and ongoing care needs
