Claratel Behavioral Health in Georgia is seeking a Housing Support Case Manager to join the Housing Support Program. You will provide case management and psychosocial rehabilitation to adults living in subsidized housing, promoting stability and independence.
You will carry a caseload of 25-30 clients, conduct weekly visits initially, coordinate services, and document in the EMR. This in-person role may require some evenings and travel within the region.
