Claratel Behavioral Health is hiring an Adult Case Manager for the Community Support Services Program. FT, Monday through Friday, 8:15 AM to 5:00 PM.
You will help adults navigate mental health services, support IRPs, and connect clients to community resources. You will conduct needs assessments, collaborate with a treatment team, coordinate care, document contacts, and promote recovery and community reintegration.
