We are currently seeking a Part-Time Transportation Aide to safely transport residents to and from scheduled appointments, including physician offices, laboratories, hospitals, dental appointments, and other approved locations. This role plays a vital part in ensuring residents receive timely and coordinated care outside the facility.

• Review scheduled appointments with the transportation scheduler daily

• Ensure residents are prepared for appointments, including personal hygiene and weather-appropriate clothing

• Obtain required paperwork and documentation from the resident’s nurse prior to departure

• Accompany residents to appointments and remain with them when family members are not present

• Transport residents safely and responsibly to and from approved locations

• Receive, understand, and accurately relay verbal instructions and information from healthcare providers to nursing staff

• Promote the safety, comfort, and well-being of all residents during transport

• Perform other duties as assigned

• Valid Illinois driver’s license required

• CNA certification preferred, but not required

• Ability to read and follow written directions

• Ability to communicate effectively in English with residents, families, and coworkers

• Demonstrated ability to interact tactfully with residents, families, staff, visitors, and external agencies

• Ability to receive, understand, and communicate detailed resident-related instructions from healthcare providers
