Salary Range:$37.50-$50.50 (Based on Experience)

• Conduct comprehensive psychosocial assessments for home health and hospice patients.
• Develop individualized care plans in coordination with nursing, therapy, and hospice teams.
• Provide counseling and support to patients and families coping with illness, decline, grief, and end-of-life needs.
• Facilitate advance care planning, including goals-of-care discussions and completion of advance directives.
• Identify and connect patients/families with community resources, financial assistance, and support programs.
• Document all assessments, visits, and interventions per CMS, state, and agency standards.
• Participate in interdisciplinary team meetings and case conferences.
• Provide crisis intervention and ongoing emotional support as needed.
• Ensure compliance with Illinois regulations and professional ethical standards.

Qualifications:

• Experience in home health, hospice, palliative care, or medical social work preferred.
• Strong communication, assessment, and documentation skills.
• Ability to work independently in a community-based setting.
• Valid driver’s license and reliable transportation.
