- Ensure coordination of home health or hospice services in accordance with standards of practice and company procedures.
- Review and approve patient information submitted by licensed professionals (LP).
- Assist with patient care review meetings (Case Conference & Interdisciplinary Team).
- Review and approve orders, provide oversight of patient care, and assist the branch director with day‑to‑day office and staff management.
- Assist branch leadership with education and training of clinicians on documentation requirements.
- Facilitate relationships with physicians, referral sources, patients, caregivers, and employees.
- Review on‑call coordination notes and communicate services to patients and families.
- Provide educational material for families and staff on medical diagnoses and end‑of‑life care.
- Maintain provider requirements and resolve issues with providers, sales, and clinical staff.
- Process workflow, coordination notes, and administrative tasks in a timely manner.
- Back up the intake coordinator and Patient Service Coordinator with referral and visit management.
- Review patient schedules, approve changes, and ensure clinical staff meet patient needs.
- Follow up on orders and ensure all orders are complete and authorized.
- Update policies, procedures, and practice guidelines, and ensure compliance.
- Provide orientation and in‑service training on documentation standards.
- Assist the Branch Director during surveys and perform administrative on‑call duties.
- Conduct continuous quality assessment, performance improvement, and onsite supervisory visits.
- Assume supervisory tasks when the branch director is absent, including evaluations, counseling, and hiring/termination recommendations.
- Manage referral intake and assessment visit scheduling.
- Attend and participate in staff meetings, community education, and action‑item reviews.
- Handle wound score deviations, vital sign alerts, lab report assessments, and infection reports.
- Review and process plans of care (POC) and ensure accurate start dates.
- Perform chart audits and maintain ongoing compliance with SOP.
- Assist with hospice item set data and lock processes as directed.
- Participate in on‑call rotation and perform any additional duties assigned by the supervisor.
- Registered Nurse (RN) with at least one year of management experience in home care, hospice, or similar environment.
- Two years of experience as an RN, with at least one year in a management role.
- Prior experience in Home Care Home Base (HCHB) is preferred.
- Proficiency in computer use preferred.
- Organized, detail oriented, able to handle multiple projects simultaneously.
- Independent work capability with minimal supervision and strong communication skills.
- High ethical standards, passion for patient care, and service‑oriented attitude.
- Knowledge of home health care delivery issues and problem‑solving ability.
- Compliance with accepted professional standards and practices.
- Graduate of an accredited School of Nursing.
- Bachelor of Science in Nursing preferred.
- Must be licensed as a registered nurse in the state of practice.
- Must pass a criminal background check and MVR check.
- Completed health statement acknowledging ability to perform duties.
- Valid state driver’s license and required automobile liability insurance.
- Current CPR card and TB testing in accordance with agency policy.
