Licensed Practical Nurse (LPN)

Travel assignment in Decatur, IL for 8 weeks, covering day and night shifts in long-term care and rehabilitation settings. Key Responsibilities:

Provide direct nursing care to assigned residents while supporting established care plans.

Document patient assessments, vital signs, medication administration, and resident responses accurately.

Maintain safe medication administration practices, including verification procedures and correct charting.

Collaborate with RN supervisors, interdisciplinary team members, and families to support quality care and resident outcomes.

Execute assigned shift duties for rotating coverage and respond appropriately to changes in resident condition.

Qualifications:

Graduate of an approved Licensed Practical Nurse (LPN) program.
Active Illinois LPN license or pending verification through the required process.
At least 1 year of recent clinical experience in long-term care and/or rehabilitation settings.
Ability to work rotating day and night shifts, including weekend coverage, in a long-term care/rehabilitation environment.
