Essential Functions- Primarily responsible for completing assessments, medical history review, monthly visits and coordinated health care for children with medical complex need who are involved in the child welfare system. Includes oversight of medical appointment, medication management, advocacy for treatment, consultation and education to foster parents, birth parents and staff via in-person contacts. May work evening and some weekends and travel to high risk communities.
- Proficient in effectively responding to and managing crisis situations, providing immediate support, and following established crisis management protocols.
- Actively assist in identifying chronic physical, developmental, dental, and mental health conditions, promptly documenting and providing referrals and collateral information as needed.
- Assist LCFS CWS and families in obtaining additional services necessary to educate and support caregivers.
- Collect health information to determine/facilitate timely linkage to referrals and services needed.
- Work with healthcare professionals and specialists to ensure children receive appropriate medical treatment and interventions.
- Maintain detailed and accurate health records, progress notes, and documentation in compliance with agency and regulatory requirements.
- Prepare reports on the health status and care plans for children in the program.
- Conduct Healthy child checkups once a quarter.
- Must spend on average one hour per month with every child in medical specialized foster care contracts within the designated region.
- Must attend well-child coordination meetings as indicated by supervisor.
- Must conduct quarterly review of coordinated care including overall report and any recommendations.
- Collaborate with child welfare teams, case workers, and legal professionals to advocate for the best interests of children's health.
- Attend case conferences, court hearings, and meetings to provide expert recommendations and support.
- Advanced knowledge of pediatric nursing principles, healthcare practices, and child health assessments.
- Proficiency in delivering health education and guidance to children, families, and professionals.
- Excellent communication skills, both verbal and written.
- Ability to maintain and manage health records and documentation in compliance with regulations.
- Ability to provide support in crisis situations.
Must hold a valid and current Registered Nurse license. Ideally possess a pediatric nursing specialization or relevant experience. Must have at least 2‑3 years of progressive experience in pediatric nursing or a related role, with experience in child health assessments.
Physical Demands and Other RequirementsTo perform this job successfully, an individual must be able to perform the essential duties: talk or hear, sit frequently, stand/walk occasionally, use hands to handle objects or controls, reach, operate office equipment, occasionally lift 20‑30 pounds, and maintain vision. Must have a valid driver’s license, auto insurance, and the ability to safely operate a motor vehicle. Noise level is low to moderate.
