CNA – Hospice Care (Decatur, MI)

The Care Team, in partnership with Traditions Health, is seeking a Certified Nursing Assistant (CNA) to join our growing Hospice Team. The primary function is to provide personal healthcare and related services to patients in their homes, ensuring a safe and clean environment while working collaboratively with patients, families, and nursing staff.

Under the direction and supervision of a registered nurse, the CNA will:

• Provide direct patient care as outlined in the Aide Plan of Care
• Assist with safe transfers and ambulation per agency policy
• Report changes in patient condition and document care provided
• Practice accepted infection‑control principles
• Maintain a clean, safe, and comfortable environment
• Promote respectful communication with patients, families, and colleagues
• Assist with proper nutrition and fluid intake
• Support efficient agency operations and demonstrate effective time management
• Participate in 12 hours of in‑service training per year
• Adhere to agency philosophy and administrative policies to ensure quality of care

Education: High school diploma preferred.

Licensure: Current driver’s license required.

Experience: At least one year as a Home Health Aide or Nursing Assistant (hospital, nursing home or home health agency) preferred.

Training: Successful completion of a state‑established or other training and competency evaluation program that meets state and federal requirements.

• Excellent written and verbal communication
• Reading, writing, and reporting clinical information accurately
• CPR certification (current)
• Proficiency with the Aide Competency Evaluation Skills Checklist and written skills test
• Ability to lift up to 50 pounds and move patients
• Hand‑eye coordination and manual dexterity

Reliable transportation with valid auto liability insurance.

• Work in patients’ homes under varying conditions, including potential exposure to blood, body fluids, and infectious diseases
• Flexible schedule with local travel required; occasional exposure to unpleasant weather
• Prolonged standing and walking; work under stressful conditions to meet deadlines and address psychosocial needs

Compensation Range: $17.88 – $21.13 per hour.
