This position will serve as a functional member of the Continuous Quality Improvement Team in implementation of program goals and objective. This role involves a systematic approach to monitoring, assessing and improving health care quality at SIU Center for Family Medicine-. Primary functions will include review of charts/processes, working reports, assisting with the assessment and monitoring of quality metrics, the teaching of preventive medicine, and assists in clinic as needed.
This position will work closely with the Director of Clinical Ops, Nurse Administrator and will collaborate with physicians, residents and other members of COi, and the health care team and patients to promote the delivery of quality, in a patient-centered environment.

Examples of DutiesClinical Services - 70%
- A. Obtains queried disease-specific and preventative maintenance reports and works lists, reviewing charts and contacting patients via phone and/or letter to notify patients in need of particular services. Assists patients with compliance of identified services and in overcoming barriers to gaps identified.
- B. Participates in clinic team activities, such as huddles or quality initiatives to communicate and share information with the health care team that will assist in both proactively addressing service needs and also assist in shoring up gaps in care.
- C. Collaborates with physicians to enhance evidence-based clinical guideline adherence and promote best practice by initiating and/or adjusting therapies and clinical interventions as directed by the physician and providing appropriate follow-up and monitoring as needed.
- D. Works with providers and support staff to assist in identifying and implementing reports or processes that improve follow-up of care, such as laboratory and diagnostic testing, utilization patterns and other metrics to monitor quality and efficiency results for assigned population.
- E. Assists in collecting/gathering data on other quality metrics that advances improvement processes such as determined by the CQI committee, relevant to UDS measures or aligning with other quality indicators.
- F. Assists with chart audits relevant to advancing population health and some subpopulations.
- G. In an interim or emergent basis, perform clinic duties and prior authorizations as assigned.
Administrative Teaching Services - 30%
- A. Assists in creating, trending and generating quality reports. Reports on recalls, FDA bulletins, drug reports, drug interactions & safety reports.
- B. Attends and reports at Quality meetings, staff meetings as requested by the Clinical Director, CQI, PCMH, and Performance Improvement Team (PIT).
- C. Works directly with the faculty/residents/staff toward improvement of identified processes. Including but not limited to UDS metrics, Care Coordination, preventive medicine
