Prairieland Service Coordination, Inc. (PSCI) is looking to welcome a Case Manager / PAS & Crisis Coordinator to our Decatur, IL office. This position focuses on completing PAS assessments, responding to crisis needs, coordinating services, and advocating for individuals with developmental disabilities and their families.

This is an in-office position based at PSCI's Decatur office. The position requires travel throughout PSCI's service area as needed to complete job responsibilities.

This position provides the opportunity to make a meaningful impact by helping individuals access necessary services and supports while ensuring compliance with Illinois Department of Human Services (DHS) requirements.

• Complete the PAS process in accordance with DHS PAS Manual requirements
• Complete crisis-related activities, E-Packets, DHS forms, and required documentation within established deadlines
• Assess individual needs and coordinate services through communication, provider linkage, service monitoring, and advocacy
• Advocate for individuals and families to ensure access to eligible and preferred services
• Schedule and complete Discovery and Personal Plan processes as part of the PAS process
• Maintain accurate individual files and documentation according to DHS and agency guidelines
• Provide problem‑solving consultation with provider agencies following DHS Conflict Resolution guidelines
• Complete daily billings and ensure accurate time reporting
• Participate in staff meetings and required trainings
• Communicate concerns, trends, and service gaps to the Administrative Program Manager
• Provide coverage and support throughout PSCI's 27‑county service area as needed

• Bachelor's degree in a human services–related field
• One year of cumulative experience providing direct services to individuals with developmental disabilities (experience may be gained through employment, volunteer work, or family caregiving)

• Strong organizational, time management, and documentation skills
• Knowledge of the developmental disabilities service delivery system, including recipient eligibility requirements and procedures
• Ability to effectively communicate and collaborate with individuals, families, provider agencies, and professionals
• Ability to manage competing priorities and meet internal and external deadlines
• Ability to work independently while also collaborating effectively with a team
• Strong problem‑solving skills and attention to detail
• Professional written and verbal communication skills
• Commitment to ethical standards and person‑centered services
• Ability to accept direction, feedback, and accountability
• Flexibility to work outside of normal business hours as needed
• Reliable transportation, a valid Illinois driver's license, and current auto insurance
