Service Coordinator
The Service Coordinator facilitates the progress of children ages birth to 3 years and their families through intake, evaluation, eligibility determination, the Individual Family Service Plan (IFSP) process, and review of progress as part of the Early Intervention (EI) service system. The Service Coordinator maintains a case load and conducts home visits to meet with families. The individual must have reliable transportation, as Service Coordinators use their personal vehicle for travel and are reimbursed for mileage at a rate equivalent to the IRS mileage rate.
This is a fast-paced position, funded by the State of Illinois, with a focus on deadlines. Strong organizational and time management skills are a must.
