Home Health Clinical Team Assistant
The Home Health Clinical Team Assistant is responsible to coordinate and maintain scheduling for all disciplines. Support scheduling, running reports and other administrative tasks to allow the field staff and clinical manager to operate at the top of their licenses and titles. Additionally, will assist in the coordination of referrals, complete necessary screening of referrals, perform insurance authorizations, and assist with hospital discharge planning.
Assist with answering and directing calls on the main home care line and communicating with staff via phone, email and/or text.
Position
Responsibilities Include:
- Understands and adheres to established policies and procedures.
- Ensures patient visits are scheduled as ordered in collaboration with Intake and Management Team in addition to clinical staff and home care aides in a timely manner.
- Establish patient schedules to provide for the continuity of care while maintaining the productivity of clinical staff and home care aides.
- Input patient and employee schedule information in the EMR in an accurate and timely manner, maintain clinical admission capacity and master schedule.
- Manage patient and employee schedules in the event of employee illnesses, emergencies, etc.
- Communicate scheduling changes between office and field staff accurately and timely.
- Accurately relays messages, cancellations and hospitalizations to clinicians/home care aides on a timely basis.
- Maintains hospitalized patient list, communicates with care team timely and performs necessary follow up.
- Provides answering service with office coverage weekly and as needed with any changes.
- Responsible for end of day confirmation calls to patients for next day admissions and communicates with Intake, clinical manager and care team any pertinent information; enters note into EMR.
- Assists with weekend and holiday Intake and scheduling coverage.
- Attends and participates with in-service education, mandatory training programs, and staff meetings as assigned including IDT meetings.
- Assists clinical manager with preparing IDT list for bi-weekly meetings and note taking during IDT.
- Performs Intake Referral functions including: capturing patient information, entry of information using Allscripts and EPIC; notify managers of referrals, issues and changes to admission plans; assigns staff to case per agency protocol; schedules patient for admission visit and other disciplines as ordered.
- Assists the program in the maintenance of patient medical record including Electronic version from Allscripts, EPIC, 4-NEXT as well as e-Faxed paperwork as appropriate and per agency policy/protocol.
- Input new patients into PING system in order to track patient across the continuum of care and discharges patient from PING upon agency DC from home health.
- Assists with incoming orders. Lab reports, etc. to make sure they get out to the clinician in a timely manner and that it gets attached to patient record in Meditech.
- Complies with HIPAA regulations and practice policies for the privacy and security of patient information.
- Assists with Managed Care authorizations as needed.
- Responsible to maintain and update forms and other correspondence and ensure the smooth flow of information across departments.
- Responsible for assisting new employees/ Monitor and assist new employees during orientation
Qualifications :
- Must possess excellent communication/interpersonal skills
- Must possess strong organizational skills and demonstrate ability to multitask
- Must be self-directed and also able to work effectively as part of a team
- Must possess excellent time management skills
- Proficient in Microsoft Office Suite (Excel, Outlook, and Microsoft Word)
- Ability to learn and document in established Electronic Medical Record system
- Minimum of a high school diploma.
- Minimum of 1-2 years in a health care setting (preferably Home Health Care) required.
