*** What You'll Do
*** As a Registered Nurse, you'll play a vital role in helping patients recover, manage chronic conditions, and remain safely at home.
*
* Responsibilities include:

*** Performing comprehensive nursing assessments in patients' homes.
* Developing, implementing, and updating individualized plans of care.
* Providing skilled nursing care, treatments, and patient education.
* Collaborating with physicians, therapists, and interdisciplinary team members to coordinate care.
* Monitoring patient progress and communicating changes in condition.
* Educating patients and caregivers to promote confidence and independence.
* Maintaining timely and accurate clinical documentation.
* Delivering care in accordance with regulatory and organizational standards.## #
*** Qualifications***###
** Required
* ** Current, unrestricted Texas Registered Nurse (RN) license
* Minimum one (1) year of nursing experience
* Strong assessment and critical thinking skills
* Valid driver's license, reliable transportation, and current auto insurance
* Ability to travel throughout the assigned service area* ##

Physical Requirements:

Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support 50–100 pounds, including assisting with patient transfers and moving equipment.###
** Preferred
* ** Home Health or Hospice experience
* OASIS documentation experience
* Homecare Homebase (HCHB) experience
* Associate or Bachelor's Degree in Nursing## # #
