Patient Educator, Specialty (RN)
The Patient Educator collaborates with medical practitioners and leaders to incorporate chronic disease education into the plan of care for a specialized group of patients. The role focuses on improving patient outcomes by promoting disease education and ensuring active patient and family involvement.
Key Responsibilities- Provide chronic disease education related to quality care for a specific patient population.
- Coordinate care between primary care physicians, community specialists and vendor services, including hospitals.
- Monitor chronic disease program performance across multiple medical centers.
- Identify gaps and implement improvements in patient and quality outcomes.
- Assist with the development of strategies for quality improvement and disease education management.
- Collaborate with medical center leaders and PCPs to evaluate, develop and implement patient education programs through needs assessment analysis.
- Educate patients and/or family members by assessing cultural, emotional, and language barriers, and readiness to learn.
- Encourage decision‑making about health care options by ensuring patients and families understand health status.
- Maximize care skills by observing patient abilities and adjusting communication as needed.
- Promote healthy lifestyles to enrich overall patient health.
- Select, adapt, and individualize patient education information based on available materials and patient context.
- Document patient care services in chart and department records.
- Study, evaluate and redesign processes; implement changes to improve quality results.
- Coordinate care activities with transitional care team and case management to decrease hospitalizations.
- Develop trusting relationships with stakeholders to identify improvement opportunities.
- Perform other duties as assigned.
- Strong business acumen and nursing knowledge.
- Excellent communication and interpersonal skills.
- Skilled in gathering, analyzing, and interpreting information.
- Customer‑service orientation and ability to work across multiple centers.
- Problem‑solving and recommendation of solutions.
- Prioritization and coordination of work activities.
- Detail oriented with ability to work independently and as part of a team.
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
- Willingness to travel locally, regionally, and nationwide up to 40%.
- Spoken and written fluency in English.
- Independent judgment usage required.
- BSN degree required.
- Minimum of 2 years specialty care nursing experience.
- Valid, active RN license required;
Nursing Compact preferred. - Teaching/training experience required (clinical or patient education preferred).
- Experience with geriatric patients desirable.
- Basic Life Support (BLS) certification required.
- Certified Nurse Educator (CNE) or similar certification a plus.
- 2 years dialysis experience.
- Pay range: $76,732 – $109,617 (base hourly or annual). Bonus or commission eligibility possible.
