We are seeking a motivated, relationship-driven Full-Time Sales & Community Liaison to represent our home care services in the community. This role focuses on building and maintaining referral partnerships with Assisted Living Facilities (ALFs), hospitals, Skilled Nursing Facilities (SNFs), rehabilitation centers, and other healthcare professionals.

• Build and maintain referral relationships with Assisted Living Facilities (ALFs), hospitals, Skilled Nursing Facilities (SNFs), rehabilitation centers, and healthcare professionals
• Conduct regular in-person visits and follow-ups with referral partners
• Educate referral sources on our services, care approach, and differentiators
• Generate and convert referrals into active clients
• Track outreach efforts, referrals, and outcomes
• Attend community networking events as needed
• Collaborate with leadership on outreach and growth strategies
• Must attend sales program offered by our professionals designed to learn sales skills and increase results.

• Hourly pay plus commission for each converted referral
• Opportunity to increase earnings based on performance

• Experience in healthcare sales, home health, senior care, or related field preferred
• Strong interpersonal and communication skills
• Self-motivated, organized, and comfortable working independently
• Professional, reliable, and relationship-driven
• Have car and a valid driver’s license
