Under the supervision of an RN and covering the Defiance area, the LPN will assess, plan, and implement patient care; educate patients and families on disease processes, procedures, and medications; and address various patient needs including wound care, traches, G/J tubes, Foley catheters, and medication administration.

The LPN will also monitor medication compliance, set up IV therapy and injections, provide education for administering injections, assist with special diets and weight monitoring, handle intravenous nutrition such as TPN and tube feeding, and carry out inhaler and oxygen equipment care. Additionally, the LPN will provide post‑operative care, perform dressing changes, wound vac therapy, and teach patients about chronic conditions such as COPD, CHF, diabetes, renal failure, multiple sclerosis, and osteoporosis.

Pediatric care may include PICC, tracheostomy, chemotherapy, parenteral nutrition, enterostomy feeding, and medication administration.

• Three years of nursing experience
• CPR certification
• Ability to travel for in‑home patient visits
