Job Overview
This is a PRN-only RNCM position with Home Care Network, Inc. We are seeking an experienced RN Case Manager to build a long‑term employment relationship by consistently completing RNCM and recertification visits, weekly medicine setups, and in‑office State Survey support (1–2 times per year) in our Perrysburg/Toledo Office, under the guidance of our Northwest Ohio Administrator.
Potential growth into a Part‑Time or Full‑Time RNCM role is possible as the agency expands over the next 6–12 months, though additional hours cannot be guaranteed at this time. Contractors or contracted arrangements are also considered.
RNs with nights‑only or weekends‑only availability cannot be considered at this time.
Employment Type& Schedule
- PRN (as‑needed) or Contractor
- Consistent weekday/daytime schedule: 1–2 weekly medication setups and 1–10 recertifications per week, Monday–Friday between 8:00 AM and 5:00 PM.
- Occasional early‑evening and rare weekend visits may be required.
- No overnight or overnight travel; no overtime shifts.
- Requests are built to fit your availability and travel preferences, prioritizing visits near your home.
- Hourly compensation paid weekly via direct deposit.
- Far‑West/Northwestern Ohio region (Defiance, Paulding & Van Worth Counties) plus a surrounding 1‑hour radius.
- Typical travel distances: 10–60 minutes; occasional travel up to 2 hours.
- Additional duties may involve expanded travel if desired.
- Required service locations include Defiance, Van Worth, Paulding; rare or occasional visits to Allen, Putnam, Henry, Williams, Mercer, Auglaize counties.
- Perform client assessments, care planning, and initiate actions in home care situations.
- Provide case management: starts of care (SOC), admissions, recertifications, transitional care, resumptions of care (ROC), clinical reviews, and other assessments.
- Conduct new hire orientations, in‑service and supervisory reviews.
- Document and communicate patient status changes to the team.
- Travel to client locations across the region and, as needed, to support State Surveys.
- Manage or supervise a multidisciplinary team of RN, LPN, Home Health Aide, and Personal Care Aides.
- Maintain high standards of safety for clients and coworkers.
- Graduate of an accredited School of Nursing.
- Minimum of one year experience as a Registered Nurse with an active Ohio RN license.
- Minimum of one year experience in a home health or acute care setting (1+ paid years required, 2+ preferred).
- Minimum of one year experience in case management.
- Experience in wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS/Non‑OASIS assessments, and excellent documentation.
- CPR and First Aid certifications required (obtainable prior to hire).
- Ability to drive and reliable transportation; current driver’s license.
- Strong background in patient care, confidentiality, and HIPAA compliance.
- Strong attendance and dependability record.

- Round‑trip mileage reimbursement.
- No on‑call requirements; no overnight shifts.

Hourly Wage: $35–$40 per hour plus door‑to‑door mileage reimbursement.
Additional InformationWe prioritize visits near your home and do not require overnight or overtime shifts. The role is suitable for nurses with flexible schedules who wish to pair this opportunity with other employment.
