** Minimum Entry Level Qualifications
** 1.
** Experience and/or level of competency:
** Minimum of 2-4 years in varied multidisciplinary settings in the health care field with sufficient experience in working with patients and families coping with illness and dying. Case management, hospice or home care experience desired. Ability to assess patient needs and formulate individualized patient care plans to meet those needs. Ability to work in a field setting and exhibit ability to make sound judgments.

Must have effective communication, comprehension, documentation, interpersonal and computer skills. Must demonstrate the ability to be able to work within a multidisciplinary team setting.

** Academic Preparation:
** Master’s Degree in Social Work (MSW) from a school of social work accredited by the Council on Social Work Education or a Baccalaureate degree phycology, sociology, social work or related area (under the direct supervision of a MSW). A current Texas Social Worker license in good standing.

* Job Summary:

** According to prescribed policies and procedures of the organization including all applicable state, federal and accreditation regulations and under the general supervision of the Director of Case Management or her/his designee the LMSW is responsible to provide patients and families psychosocial care regarding emotional, social and economic needs under their hospital and/or hospice individualized plan of care. The LMSW will provide bereavement counseling to help family members and significant others cope with grief and loss issues related to death, for a minimum period of 13 months following the death of a hospice patient.

The LMSW will develop, coordinate, supervise, implement and document the provision of the Bereavement Plan of Care related to the bereavement needs of families/significant others. The LMSW works in collaboration with all members of the healthcare team and provides education on social service related issues. All social services provided are documented timely into the appropriate electronic health record. Database to ensure compliance with current standards and policies.
** Specific Responsibilities:
** 1. Assesses the psychosocial status of patient/family as it relates to capacity to cope with diseases, terminal illness, environment and death. Provides counseling services as applicable and assists in utilizing community Communicates findings to the team.
2. Participate in the development of the plan of care and treatment for inpatients and hospice patients. Holds case conferences as needed.
3. Provides discharge care coordination and follow-up assessment of patients deemed high-risk for readmission or high ED utilization.
4. Prepare social histories and evaluation and plans interventions based on their findings being sure to look at:
1. Family   2. The meaning of illness to the patient and his/her family   3. The cultural attitudes in relationship to illness   4. The meaning of home to the patient   5. The ability of the patient and his family to adjust to the patient at home   6. The financial impact of illness upon the family
5. Assess, evaluates and assists with patient’s financial obligations, assessing appropriate resources, and insurance coverage.
6. Observes, records and reports information on patient’s condition to the attending physician and in the patient record. Submits documentation accurately and timely. Updates and maintains client's records and charts.
