You will coordinate patient care, support families, and work with an interdisciplinary team to deliver compassionate, patient‑centered services. A $5,000 sign‑on bonus is offered for eligible candidates.
The role features a full-time schedule, Monday–Friday 8AM–5PM, and travel across Waukesha and Milwaukee Counties. Prior hospice experience and Wisconsin licensure are preferred.
