Job Description:Performs initial assessments to evaluate patient needs related to diabetes management. Develops personalized educational plans for diabetes management and wellness. Provides diabetes education for inpatients and outpatients through private sessions and group classes. Monitors patient progress and provides ongoing support and education. Collaborates with healthcare professionals to ensure comprehensive diabetes care. Stays updated on best practices in diabetes management and wellness programming.
Participates in community outreach to promote diabetes awareness and wellness initiatives. Promotes wellness initiatives to improve patient outcomes related to diabetes. Engages in diabetes management activities, including monitoring and support. Creates personalized education plans for diabetes management. Accurately documents patient information related to diabetes care. Provides staff training on diabetes care and treatment. Other duties as assigned.
- Knowledge of diabetes of diabetes concepts and skills [Required]
- Ability to provide patient teaching in individual and group settings [Required]
- Ability to communicate effectively and organize daily activities [Required]
- Knowledge of computer programs such as Microsoft Word, Power Point and Microsoft Outlook. [Required]
- Ability to work with people of various backgrounds. [Required]
- Ability to work under stress and deadlines. [Required]
- Advanced knowledge of Diabetes [Preferred]
- Ability to provide teaching in all aspects of diabetes management [Preferred]
- Bilingual in Spanish [Preferred]
- Bachelor's [Required]
- in dietetics or nursing
- 1+ experience in diabetes specialty [Preferred]
- 1+ of experience in field [Required]

Certifications:
- Basic Life Support - CPR Cert (BLS) [Required]
- Registered Dietitian Nutritionist (RDN) [Required]OR Registered Nurse (RN) [Required]
- Certified Diabetes Care and Education Specialist (CDCES) [Preferred]

Pay Range:$28.68 - $53.35
