- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- CHPN certification (Preferred)
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience

Roles & Responsibilities
- Assess patients in home health and/or hospice settings to identify their physical, psychosocial, and environmental needs.
- Implement, document, and evaluate the Plan of Care to ensure continuity and quality of care.
- Educate patients and families regarding care routines, medications, and home safety.
- Provide clinical direction and supervision to Home Health Aides to ensure quality care.
- Arrange referrals to additional services as needed and monitor compliance with third-party payer requirements.
- Communicate effectively with patients, families, and healthcare team members to facilitate care coordination.
