Center Well Home Health is seeking a nurse to provide one-on-one in-home care under the supervision of an RN. You will collaborate with physicians and clinicians to manage care plans, educate patients and families, and document visits and outcomes.
Ideal candidates hold an active nursing license, CPR certification, and a valid driver's license with reliable transportation. Two years of LPN/LVN experience in a clinical setting is required, with a Home Health or Hospice background preferred.
