Role DescriptionThe Bridge Shelter Case Manager is a full-time, on-site position based in DeLand, FL. This role is responsible for providing direct case management to shelter guests, including intake assessments, individualized service planning, goal setting, and regular follow-up to support progress toward stable housing. The Case Manager coordinates referrals to community resources such as healthcare, mental health services, substance use treatment, employment programs, and benefits assistance.
Daily activities include maintaining accurate documentation, updating case notes, and tracking outcomes in accordance with organizational and funding requirements. The position also involves collaborating with shelter staff and partners to support a safe, respectful environment, participating in case conferences, and responding to guest needs and crises in a professional, trauma-informed manner.
- Strong case management skills, including client assessment, service planning, resource coordination, and follow-up.
- Knowledge of homelessness, housing instability, and related social services, with the ability to navigate community resources and referral networks.
- Excellent communication and interpersonal skills, including active listening, conflict de-escalation, and the ability to build trust with diverse populations.
- Organizational and administrative abilities, including accurate documentation, time management, and use of case management or data entry systems.
- Commitment to trauma-informed, person-centered, and culturally responsive care, with a nonjudgmental and empathetic approach.
- Ability to work collaboratively in a team environment and maintain professional boundaries while providing support.
- High school diploma or equivalent required; associate or bachelor’s degree in social work, human services, psychology, or a related field preferred.
- Prior experience in shelter, social services, or related community work is strongly preferred.
- Ability to work on-site in DeLand, FL, with flexibility for occasional evenings or weekends as program needs require.
- Basic computer proficiency (email, word processing, data entry) and willingness to learn agency-specific software.
