Position Summary:The Neighborhood Center of West Volusia (NHCWV) Prevention Services Case Manager will assist residents of Volusia County using agency-managed projects and programs that enable clients to prevent homelessness and associated issues. This role involves assessing client needs, developing personal financial plans, and connecting clients with resources to improve their financial health. The Case Manager has strong analytical skills, excellent communication abilities, and a passion for helping individuals achieve holistic wellness and prosperity.
ServiceCoordination
- Conduct comprehensive assessments of prospective clients’ financial situations, including income, expenses, debts, and assets; current crisis that requires financial assistance; and actionable short-term and long-term financial goals.
- Verify the client’s eligibility for, and funding availability, of various NHCWV grants, to determine which program best meets the clients’ needs.
- Support prospective clients in creating customized Individual Service Plans (ISP) based on current needs and assets that emphasize budgeting, debt reduction, and money savings strategies.
- Recommend financial products and services as needed, while ensuring that any financial advice complies with relevant laws, regulations, and agency policies.
- Facilitate referrals to supportive programs and community resources.
- Regularly follow-up with clients to track progress toward financial goals and adjust plans as needed, up to one year after program entry.
- Ensure ongoing financial education by referring clients to workshops and events in the community, or facilitating one-on-one education meetings.
- Assist the Prevention Services department in facilitating new client intake assessments, explaining processes and procedures, and referring to internal agency services within other departments, as applicable.
- Engage in ongoing professional development about financial resources, tools, and methods to assist clients.
- Maintain detailed and accurate records of client intake documents, client interactions, assessment interviews, and progress reports within the records database.
- Maintain client confidentiality in accordance with agency and funder policies.
- Prepare and communicate client outcomes and program effectiveness reports for the Prevention
- Collaborate with volunteers and employees to ensure cohesive delivery of services and client care.
- Support Prevention Services department operations by assisting in planning and facilitating special events, including, but not limited to: bi-monthly Mobile Food Drop grocery giveaways;
Summer, Thanksgiving, and Christmas food distribution; and winter holiday toy giveaways. - Receive cross-training to facilitate food distribution through the Choice Pantry in accordance with food safety and partner agency standards.
