General SummaryThe Transitions of Care (TOC) Clinical Pharmacist plays a critical role in supporting the safe, effective, and seamless movement of patients across care settings from the hospital stay to being discharged. The pharmacist collaborates closely with interdisciplinary healthcare providers, including physicians, nursing team members, social workers, and care managers to conduct discharge medication reconciliations and optimize medication-related care.
The TOC Pharmacist also works in partnership with Bayhealth inpatient, specialty and community pharmacy services to coordinate medication orders, delivery, and patient counseling upon discharge.
Responsibilities- Deliver comprehensive clinical pharmacy services including assessment, monitoring, and optimization of individualized pharmaceutical therapy.
- Perform discharge medication reconciliation in collaboration with providers, ensuring continuity and accuracy of medication regimens.
- Support the Meds to Beds program through coordination of discharge medication orders, delivery logistics, and patient counseling.
- Provide post-discharge follow-up phone calls and medication assessments for patients with a high risk of readmission to prevent adverse drug events.
- Act as a clinical resource to healthcare professionals, offering education and expertise on medication management in transitions of care.
- Assist in the development, implementation, and adherence to protocols and policies related to discharge planning and medication use.
- Provide drug monitoring services and medication-related recommendations to ensure safe and evidence-based therapy.

- Education:
Bachelor Degree Pharmacy - Credential(s):
Licensed Pharmacist - Experience:
Three (3) years of any pharmacy practice experience.

Other - See Equivalency PGY1 Pharmacy Residency - Experience:
Preferred:
One (1) year or more of hospital pharmacy practice experience.
