Physical Therapist Assistant (PTA)

Job Summary:

Polaris Health Care and Rehabilitation Center is seeking a skilled and compassionate Physical Therapist Assistant to join our rehabilitation team. As a per diem employee, you will provide therapy services to patients in our skilled nursing facility, helping them recover their independence and reach their full potential.

Responsibilities:

  • Implement treatment plans established by the physical therapist to restore function and performance

  • Provide dynamic, creative, and challenging treatments to patients to facilitate progress and improvement

  • Monitor patients' performance in therapy activities and adjust the treatment program to obtain improved progress

  • Collaborate with the interdisciplinary therapy team members to provide input regarding patient needs and establish a rehabilitation plan

  • Maintain positive interaction with patients, families, and caregivers while providing instruction and education for the skills and functions essential for promoting patient independence in collaboration with the supervising physical therapist

Qualifications:

  • Experience providing physical therapy to patients in a skilled nursing facility

  • Graduated from an accredited physical therapy assistant program

  • Current Physical Therapy Assistant license for the state of Delaware

  • Current CPR certificate

  • Must have good organizational skills, time management skills, and good verbal/written communication skills
