We are seeking a compassionate and dedicated HHA to join our team at PACE in Milford, Delaware. As part of our full-time team, you will provide exceptional care and support to our patients in their homes, focusing on their physical, emotional, and social well-being. • Provide direct patient care, including personal care, wound care, and vital sign monitoring
• Assist patients with daily living activities, such as bathing, dressing, and grooming
• Support patients with medication management, including administration and tracking
• Maintain accurate and detailed records of patient care, treatment plans, and progress
• Collaborate with healthcare providers and families to ensure comprehensive care
• Demonstrate a strong understanding of patient rights and dignity, respecting individual preferences and needs

• High School Diploma or equivalent
• Nice to have current certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) in the state of Delaware
• Ability to lift, transfer, and reposition patients according to established guidelines
• Excellent communication and interpersonal skills
• Organizational skills with attention to detail and accuracy
• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently
