Vital Core Health Strategies (VCHS), an industry leader in Correctional Health Care, is hiring a Full-Time Telehealth Coordinator/ Chronic Care Registered Nurse at Howard R. Young Correctional Institution in Wilmington, DE.
Role SummaryThe RN Telehealth Coordinator facilitates virtual medical consultations and appointments for patients who require specialized care beyond the facility's capabilities. They monitor patients from intake through release, bridging gaps between multiple systems to ensure continuity of care and advocate for patient needs.
Responsibilities- Maintain accurate, detailed reports and records, including medical histories, test results, and treatment plans.
- Document medical histories, test results, treatment plans, and communication with external healthcare providers.
- Administer medications and monitor patients for reactions and side effects.
- Monitor, record, and report symptoms or changes in patients’ condition.
- Coordinate with outside healthcare providers, schedule appointments, and ensure necessary medical records are prepared.
- Perform administrative tasks such as scheduling appointments and maintaining records.
- Complete additional duties as assigned by the facility.
- Graduate of an accredited School of Nursing.
- Active licensure as a Registered Nurse in the state of employment (Delaware or multistate).
- Active CPR certification.
- Knowledge of state laws and regulations governing nursing practice.
- Completion of initial and annual clinical competencies as assigned by the role.
Day Shift – 8-hour day.
